Makhzan Orthodontics  |  https://makhzanorthodontics.com/

Effective Date: November 05, 2026

These Website Terms of Service (“Terms”) govern your use of https://makhzanorthodontics.com/ (the “Website”), operated by Makhzan Orthodontics (“Practice,” “we,” “us,” or “our”). By accessing or using the Website, you agree to these Terms. If you do not agree, do not use the Website.

1. Informational Purposes; No Medical Advice

The Website is provided for general informational and educational purposes. Website content is not medical or dental advice, diagnosis, or treatment and is not a substitute for an evaluation by a qualified health care professional. Do not delay or disregard professional care because of information on this Website.

If you are experiencing a medical or dental emergency, call 911 or seek appropriate emergency care. Do not use the Website, email, or text messaging for urgent or emergency communications.

2. No Provider-Patient Relationship

Using the Website, submitting a contact or appointment form, sending an email or text message, or otherwise communicating through the Website does not by itself create a provider-patient relationship. A provider-patient relationship is established only when the Practice accepts you as a patient and the applicable clinical and intake requirements are completed.

3. Eligibility and Use by Minors

The Website is intended primarily for adults and for parents or legal guardians seeking information or services for minors. A minor should use interactive Website features only with the involvement or permission of a parent or legal guardian, as appropriate.

4. Appointment Requests and Online Forms

An appointment request submitted through the Website is only a request. An appointment is not confirmed until the Practice confirms it. We may contact you to obtain additional information, schedule, reschedule, or cancel an appointment as reasonably necessary.

Unless a Website form specifically states that it is intended for secure transmission of health information, do not submit highly sensitive medical or dental information through a general contact form or ordinary email. Use the patient portal or another method directed by the Practice when available.

5. Privacy and Health Information

Your use of the Website is also subject to our Website Privacy Policy. Protected health information (“PHI”) maintained by the Practice is handled in accordance with applicable health privacy laws, including HIPAA when applicable, and our Notice of Privacy Practices. If there is a conflict between the Website Privacy Policy and the Notice of Privacy Practices with respect to PHI, the Notice of Privacy Practices and applicable health privacy law control.

6. SMS and Mobile Communications

If you provide a mobile phone number and opt in to receive text messages from Makhzan Orthodontics, you may receive messages concerning appointment reminders, scheduling, care-related information, billing or account information, practice announcements, and other communications you requested. Marketing or promotional text messages will be sent only when permitted by law and, when required, after obtaining the appropriate consent.

  • Consent to receive text messages is not a condition of purchasing treatment or services.
  • Message frequency varies. Message and data rates may apply.
  • You may opt out of recurring text messages at any time by replying STOP. You may receive one final confirmation message after opting out.
  • For help, reply HELP or contact us at 770-901-2222 or info@makhzanortho.com.
  • Wireless carriers are not responsible for delayed or undelivered messages.
  • Do not use text messaging for emergencies or urgent clinical concerns.

Your carrier, mobile operating system, messaging provider, and other technology providers may process information associated with text messaging according to their own terms and privacy practices.

7. Acceptable Use

You may use the Website only for lawful purposes. You may not:

  • submit false, misleading, fraudulent, or unlawful information;
  • attempt to gain unauthorized access to the Website, systems, accounts, or data;
  • introduce malware, malicious code, or other harmful material;
  • interfere with the Website’s operation, security, or availability;
  • scrape, harvest, or use automated means to access the Website in a manner that violates applicable law or materially burdens the Website; or
  • use the Website to violate the rights of the Practice or any other person.

8. Intellectual Property

Unless otherwise stated, the Website and its content—including text, graphics, photographs, videos, logos, design elements, and other materials—are owned by or licensed to the Practice and are protected by applicable intellectual property laws. You may view and use Website content for personal, noncommercial purposes. You may not reproduce, distribute, modify, publicly display, or commercially exploit Website content without permission except as allowed by law.

9. Third-Party Websites and Services

The Website may link to or integrate with third-party websites or services, such as patient portals, payment processors, maps, social media platforms, scheduling tools, or other vendors. Third-party services are governed by their own terms and privacy practices. We do not control and are not responsible for third-party content, availability, security, or privacy practices, except to the extent applicable law imposes responsibilities on us.

10. Website Availability and Accuracy

We work to keep Website information useful and current, but we do not guarantee that all content will always be complete, current, error-free, or continuously available. Treatment options, office hours, insurance participation, fees, promotions, and other information may change. Contact the Practice to confirm information that is important to your care or financial decisions.

11. Disclaimer of Warranties

To the fullest extent permitted by law, the Website and its content are provided on an “as is” and “as available” basis without warranties of any kind, express or implied. Nothing in this section disclaims any warranty or obligation that cannot lawfully be disclaimed.

12. Limitation of Liability

To the fullest extent permitted by law, the Practice and its owners, employees, contractors, and agents will not be liable for indirect, incidental, special, consequential, or punitive damages arising solely from use of or inability to use the Website. This provision does not limit liability that cannot be limited under applicable law and does not alter any rights or obligations arising from clinical treatment, professional services, or separate patient agreements.

13. Changes to the Website or These Terms

We may modify the Website or these Terms from time to time. Updated Terms will be posted on the Website with a revised effective date. Your continued use of the Website after updated Terms are posted constitutes acceptance of the updated Terms to the extent permitted by law.

14. Governing Law

These Terms are governed by the laws of the State of Georgia, without regard to conflict-of-law principles, except to the extent federal law applies. Any court proceeding concerning these Terms must be brought in a court of competent jurisdiction in Georgia unless applicable law requires otherwise. These Terms do not replace any separate patient, financial, informed-consent, or other agreement between you and the Practice.

15. Severability

If any provision of these Terms is found invalid or unenforceable, the remaining provisions will remain in effect to the fullest extent permitted by law.
